Eosinophilic cholangitis (EC) is a rare condition characterized by eosinophilic infiltration of the bile ducts, often mimicking diseases like primary sclerosing cholangitis or cholangiocarcinoma. We report the case of a 32-year-old man with severe epigastric pain and elevated liver function tests. Initial imaging revealed common bile duct dilation and multiple strictures, initially suggestive of primary sclerosing cholangitis.

Lemmel's syndrome is a rare condition characterized by obstructive jaundice due to the periampullary duodenal diverticulum in the absence of choledocholithiasis or tumors. Its infrequent occurrence and non-specific clinical presentation can make it difficult to distinguish from other conditions. We present a case of Lemmel's syndrome in a 63-year-old male who exhibited symptoms of abdominal pain in right hypochondrium, vomiting, and fever.

Objective: To investigate whether preoperative biliary drainage is beneficial for patients undergoing pancreaticoduodenectomy.
Methods: The PubMed, Cochrane Library and the Web of Science were systematically searched for relevant trials that included outcome of pancreaticoduodenectomy with and without preoperative biliary drainage from January 2010 to May 2024. The primary outcomes are postoperative pancreatic fistula and intra-abdominal infection.
The EUS-guided rendezvous technique was performed for stent implantation.

When Endoscopic retrograde cholangiopancreatography (ERCP) fails in patients with obstructive jaundice, EUS-guided rendezvous technique (EUS-RV) becomes an alternative treatment. For the EUS-RV, we can puncture the intrahepatic bile duct (IHBD) and the extrahepatic bile duct (EHBD). Puncturing the EHBD needs through duodenum, the puncture site can be divided into via the proximal duodenum (D1) and via the second portion of the duodenum (D2).
