Department of Otorhinolaryngology, Queen Alexandra Hospital, Portsmouth, UK.

Published: October 2008

Objective: Carcinoma cuniculatum is a rare variant of low-grade squamous cell carcinoma. We report the second case of carcinoma cuniculatum of the larynx in the literature.

Method: Case report and review of the world literature concerning carcinoma cuniculatum of the larynx and upper aerodigestive tract.

Results: A histologically proven carcinoma cuniculatum of the larynx is reported in a 72-year-old man. The patient underwent a supracricoid laryngectomy with crico-hyoidopexy, and was free of disease at 70 months after surgery. We emphasise the clinical presentation, histology and therapeutic approach of this rare tumour.

Conclusion: To our knowledge this is the second report in the world literature of carcinoma cuniculatum of the larynx. Carcinoma cuniculatum of the larynx must be considered as a distinct 'clinicopathological entity' and close cooperation between the clinician and the pathologist is essential for the correct diagnosis of these tumour as regards to the correct classification and therapy.

Article Synopsis
  • * This review examines existing research on the prognostic implications of HNSCC’s histopathologic subtypes, noting a lack of studies focusing specifically on treatment outcomes for each subtype.
  • * The review highlights that some subtypes, like verrucous and cuniculatum, have better prognoses compared to others like basaloid and spindle cell, and it emphasizes the need for more molecular research to develop targeted therapies and improve prognostic assessments.

Carcinoma cuniculatum is a variant of squamous cell carcinoma, characterized by a slow growth with progressive crypt-like invasion of deep tissue. This tumor is frequently misdiagnosed as a benign skin lesion both clinically and histopathologically. The final diagnosis is often delayed as it requires a large sample biopsy.

Background: Oral carcinoma cuniculatum (CC) is a rare variant of squamous cell carcinoma. It exhibits a locally aggressive behaviour, and its histopathology poses a diagnostic challenge, as it can be mistaken for other conditions. We present five cases of oral CC treated at our institution and conduct a comprehensive review of the current literature.

It is a rare and slow-growing tumor with a peculiar infiltrative growth pattern resembling rabbit burrows (cuniculi). It usually occurs over the plantar aspect of the foot but can also occur at other sites like the oral cavity and genitals.
