Objective: Increased intramuscular and subcutaneous tissue pressures are often found in patients with severe chronic venous insufficiency venous ulcer disease. Additional subcutaneous para-tibial fasciotomy promotes early ulcer healing. This study evaluates the mid-term effect of eradication of superficial reflux with additional fasciotomy in patients with increased tissue pressures.

Paratibial fasciotomy has been employed since 1981 to treat patients with chronic venous insufficiency (CVI) and therapy-resistant leg ulcers with severe lipodermatosclerosis. The characteristic morphological changes in CVI were evaluated shown before and after paratibial fasciotomy using computerized tomography (CT) and magnet resonance tomography (MRT). 10 patients (6 female, 4 male) were examined by CT and MRT pre- and postoperatively.

Since 1981 the paratibial fasciotomy is being increasingly used in the treatment of chronic venous congestion with persisting ulcers and severe dermatosclerosis. We now overlook more than 1800 successful operations. In a prospective study on patients who underwent paratibial fasciotomy, we are still finding a high healing-quotient in respect to the leg ulcers.
