(1) Single injections of penicillin in saline administered intramuscularly with ice applications at the site of the injection produced and maintained bacteriostatic levels for six to twelve hours in nine out of ten patients, nine of whom were diagnosed as having gonorrhea and one urethritis with extracellular diplococci. (2) There was one failure, Subject No. 10, who received only 30,000 units. (3) Eight of the patients with gonorrhea and the one patient with urethritis were cured (bacteriologically and clinically) by this single intramuscular injection of penicillin.

Antimicrobial resistance (AMR) is a major threat to global public health that continues to grow owing to selective pressure caused by the use and overuse of antimicrobial drugs. Resistance spread by plasmids is of special concern, as they can mediate a wide distribution of AMR genes, including those encoding extended-spectrum -lactamases (ESBLs). The CTX-M family of ESBLs has rapidly spread worldwide, playing a large role in the declining effectiveness of third-generation cephalosporins.

This quality improvement study aimed to enhance History and Physical (H&P) note documentation and charge capture processes to integrate coding and billing aspects, capture authentic work, preserve the H&P's integrity and align H&P-related revenue with actual performance. A multidisciplinary team, including divisional leadership and specialists in documentation improvement, electronic health records, lean/six sigma methodology, a nocturnist and a senior-level physician coding auditor, initiated a quality improvement project.
