[Double lip treated with "midmoon" incision. Report of a case].

Published: December 2007

A double-lip is an anomaly which may be either congenital or acquired and may occur either isolated or as a component of Ascher's syndrome. The upper lip is involved more often than the lower, although on occasion both may be involved. Apart from a deformity that interferes with mastication and speech, surgical treatment may be indicated for cosmetic reasons. Various surgical techniques to correct the double lip have been described: elliptical incision, w-plasty, triangle incision and elliptical incision on each side, combined with a vertical midline z-plasty to release the lip's central constriction. In the case presented a long midmoon incision was used, preserving the maxillary labial frenum. Satisfactory function and aesthetic results were achieved in this patient.

Patients seeking reconstructive facial surgery desire aesthetically pleasing outcomes with minimal visible scarring. For scalp incisions, beveled angle of 45° is typically preferred to preserve hair follicles. However, no consensus exists regarding the ideal incision angle for the brow area.

Article Synopsis
  • The study investigates the effectiveness of two surgical techniques (Wise pattern vs. modified vertical technique) for post-mastectomy reconstruction in patients with severe breast ptosis.
  • It highlights that while both methods had similar BMI among patients, the Wise pattern showed a higher incidence of complications, such as seroma and skin necrosis.
  • The findings suggest that the vertical pattern might be a safer and simpler option for certain patients, potentially reducing the risk of complications.

Simple incision and suture modeling on fixed structured grids.

J Mech Behav Biomed Mater

December 2024

Technical University of Denmark, Department of Civil and Mechanical Engineering, Nils Koppels Allé, Building 404, Kongens Lyngby, 2800, Denmark.

This paper presents a simple yet versatile approach to simulate skin incisions and suturing using nonlinear finite element analysis on a fixed structured grid with a spring-based suture model. Incisions and wounds are introduced by element removal and the sutures are discretely modeled by applying linear constraint relations pairwise to groups of nodes. Two numerical examples utilize the closing of an elliptical wound to study the influence of the number of sutures and their location, and two test cases use the Z-plasty transposition flap technique to investigate the versatility of the proposed method and for comparison with state-of-the-art results.

Breast ptosis presents challenges for implant-based reconstruction due to the large skin envelope. Skin-reducing mastectomy reduces the envelope but must consider many other factors including complications. Limited data exist on incision impact on outcomes.

Masculinization of the Chest in Transmen with Large Breasts.

Indian J Plast Surg

April 2024

Department of Plastic, Reconstructive, Facio-maxillary, Aesthetic Surgery, Madras Medical College, Chennai, Tamil Nadu, India.

Surgery to masculinize the chest is a frequently sought-after procedure by transmen who wish to conform their physical appearance to their gender identity. In our study, the outcomes of surgical masculinization comprising subcutaneous mastectomy through a modified elliptical incision, liposuction, reduced inert nipple-areola grafting, and quilting stitches were analyzed.  Thirty-two transmen with large breasts and hypertrophied nipples and areolae underwent the above-mentioned masculinizing procedure in this prospective cohort study.
