The authors present the results of endoscopic microdiskectomy (EMD) after J. Destandau in the treatment of lumbar disk herniation in 172 patients. After surgery, radicular pain regressed in all the patients. During EMD, dural damage with liquorrhea occurred in 4 patients. Spondylodiscitis developed in 2 patients. The mean duration of hospital treatment was 5 days. EMD could decrease the sizes of a skin incision and minimize traumatism of paravertebral soft tissues and nerve structures. It reduced the time of hospital treatment. The results of EMD are comparable with those of microsurgical diskectomy.

Case: A 73-year old man who underwent previous L2-S1 decompression presenting with new right radicular leg pain. Imaging suggests a large central disk herniation at L1-2 with possible intrathecal extension requiring surgical decompression. When positioned prone on a Jackson frame, neuromonitoring motor signals became diminished, and thus, the case was aborted.

Introduction: Increased intracranial pressure (ICP) is common with viral encephalitis in children which is associated with complications and prognosis. The optic nerve sheath diameter (ONSD) is a new indicator for the assessment of intracranial pressure using ultrasound, CT scan and MRI imaging. Given the influence of physical development on ONSD size in children, we expect more accurate assessment of intracranial pressure with ONSD/ETD (eyeball transverse diameter) ratio by ultrasound.
