Anesthetic practice in Japan: past, present, and future.

Significant progress has been made over the past 20 years in the development of anesthetic agents and anesthetic monitors. Due to progress in the development of medical devices and the establishment of an effective medical system, advanced age alone is no longer a contraindication for surgery. However, despite approval of the volatile anesthetic sevoflurane, Japan is lagging behind other developed countries with regard to anesthetic agents. The ultrashort-acting opioid analgesic remifentanil has finally become available in Japan, and the fast-onset neuromuscular blocking agent rocuronium is expected to be approved soon. Patient recruitment for phase III clinical trials using sugammadex, a selective reversal agent for rocuronium and vecuronium, has been completed in all countries, including Japan. In this article, changes in anesthesia over the past two decades are described and possible future changes in anesthesia in Japan are discussed.

Esketamine, a newly developed antidepressant, is the subject of this research which seeks to explore its impact on depressive symptoms in neuropathic pain mice and the potential molecular mechanisms involved. Through transcriptome sequencing and bioinformatics analysis combined with in vivo studies, it was identified that esketamine markedly boosts the levels of the m6A methyltransferase METTL3 and the AMPA receptor GluA1 subunit. Esketamine activates METTL3, allowing it to bind with GluA1 mRNA, promoting m6A modification, thereby enhancing GluA1 expression at synapses.

Recreational use of nitrous oxide (NO) has risen dramatically over the past decades. This study aimed to examine its rewarding effect and the underlying mechanisms. The exposure of mice to a subanesthetic concentration (20%) of NO for 30 min for 4 consecutive days paired with NO in the morning and paired with the air in the afternoon produced apparent rewarding behavior in the conditioned place preference (CPP) paradigm.

Rationale: Fentanyl and fentanyl analogs continue to pose a serious threat to the public health. The vast number of fentanyl analogs emerging on the black-market call for optimized analytical methods for the detection, analysis, and characterization of these extremely dangerous drugs.

In this study, we aimed to explore the sex-specific effects and mechanisms of sevoflurane exposure on the neural development of pubertal rats on the basis of M1/M2 microglial cell polarisation and related signalling pathways. A total of 48 rat pups (24 males and 24 females) were assigned to the 0- or 2-h sevoflurane exposure group on the seventh day after birth. The Morris water maze (MWM) test was subsequently conducted on the 32nd to 38th days after birth.
