Background: Electronic health records (EHRs) have been proposed as a sustainable solution for improving the quality of medical care. We assessed the association between EHR use, as implemented, and the quality of ambulatory care in a nationally representative survey.
Methods: We performed a retrospective, cross-sectional analysis of visits in the 2003 and 2004 National Ambulatory Medical Care Survey. We examined EHR use throughout the United States and the association of EHR use with 17 ambulatory quality indicators. Performance on quality indicators was defined as the percentage of applicable visits in which patients received recommended care.
Results: Electronic health records were used in 18% (95% confidence interval [CI], 15%-22%) of the estimated 1.8 billion ambulatory visits (95% CI, 1.7-2.0 billion) in the United States in 2003 and 2004. For 14 of the 17 quality indicators, there was no significant difference in performance between visits with vs without EHR use. Categories of these indicators included medical management of common diseases, recommended antibiotic prescribing, preventive counseling, screening tests, and avoiding potentially inappropriate medication prescribing in elderly patients. For 2 quality indicators, visits to medical practices using EHRs had significantly better performance: avoiding benzodiazepine use for patients with depression (91% vs 84%; P = .01) and avoiding routine urinalysis during general medical examinations (94% vs 91%; P = .003). For 1 quality indicator, visits to practices using EHRs had significantly worse quality: statin prescribing to patients with hypercholesterolemia (33% vs 47%; P = .01).
Conclusion: As implemented, EHRs were not associated with better quality ambulatory care.

Legume content (LC) in grass-legume mixtures is important for assessing forage quality and optimizing fertilizer application in meadow fields. This study focuses on differences in LC measurements obtained from unmanned aerial vehicle (UAV) images and ground surveys based on dry matter assessments in seven meadow fields in Hokkaido, Japan. We propose a UAV-based LC (LC) estimation and mapping method using a land cover map from a simple linear iterative clustering (SLIC) algorithm and a random forest (RF) classifier.

The geochemical and chemical constituents of river water quality could be influenced by human activities and organic processes like water interacting with the lithogenic structure that the river flows through. Evaluating evidence based primary root of the predominant pollutant ions, their interactions as well as the factors controlling their dominance is crucial in studies regarding water environment and hydrology especially as most studies focus on theoretical methods. In order to understand the water cycle, safeguard surface water resources, and preserve the human environment, this study evaluated surface water hydro-chemical facies, quality dynamics, and portability in southern Nigeria using multivariate statistical approaches by analyzing selected hydro-chemical characteristics as indicators of pollution along the river during wet and dry seasons.

Reservoir-operation optimisation is a crucial aspect of water-resource development and sustainable water process management. This study addresses bi-objective optimisation problems by proposing a novel crossover evolution operator, known as the hybrid simulated binary and improved arithmetic crossover (SBAX) operator, based on the simulated binary cross (SBX) and arithmetic crossover operators, and applies it to the Non-dominated Sorting Genetic Algorithms-II (NSGA-II) algorithm to improve the algorithm. In particular, the arithmetic crossover operator can obtain an optimal solution more precisely within the solution space, whereas the SBX operator can explore a broader range of potential high-quality solutions.

Medium- and long-chain triacylglycerols (MLCTs) are regarded as healthy premium oils; however, the health benefits of novel MLCTs enriched with lauric and α-linolenic acids are still not fully understood. This study examined the health benefits of lauric-α-linolenic structural lipids (ALSL) and physical mixture (PM) with a similar fatty acid composition in mice with obesity induced by the high-fat diet (HFD). The data indicated that ALSL is more effective than PM in counteracting obesity, insulin resistance, hyperlipidaemia, liver injury, and systemic inflammation in HFD-induced mice.
