We present the case of a 74-year-old female patient with a 50 mm ascending aortic aneurysm who underwent ascending aorta replacement. During routine open heart surgery, suboptimal flow in the cardiopulmonary bypass circuit, led to the discovery of a type B aortic dissection with substantial flow in the false lumen. Conservative management was chosen, focusing on blood pressure control in the ICU.

With the advancement in imaging technology, ECG-gated cardiac multidetector computed tomography (MDCT) has emerged as a tool for the anatomic evaluation of the pulmonary valve and right ventricular outflow tract (RVOT) in human medicine. Biventricular assist devices (BiVAD) are capable of providing mechanical support to the left and right ventricles to improve blood supply in heart failure, thereby maintaining patients' lives and improving their quality of life. But there is evidence that the incidence of aortic valve incompetence and other valvular pathologies is related to BiVAD support. Such as constant speed (CS) control may cause the valve to close completely and lose its normal valve function.

Background: In patients with mechanical aortic and mitral valves requiring catheter ablation of ventricular tachycardia (VT), a technique for access from the right atrium (RA) to the left ventricle (LV) via puncture of the inferoseptal process of the LV was previously described in a single-center series.
Objectives: This study sought to report the multicenter experience of VT ablation using this novel LV access approach.
Methods: We assembled a multicenter registry of patients with double mechanical valves who underwent VT ablation with RA-to-LV access.
