In PCL reconstruction many techniques have been described. Crucial item in pre-operative planning is graft choice and graft fixation. Compared to other grafts, hamstring is getting more popular because it is longer, has higher mechanical properties and doesn't harm extensor apparatus of the knee. Furthermore the absence of the bone block makes easier hamstring insertion in the femoral tunnel. Hamstring greater disadvantage is fixation. Many fixation devices, used in ACL reconstruction, have been described for PCL procedure. One of these, recently proposed, is a cross-pin femoral fixation technique, for hamstring PCL reconstruction [10]. The authors drilled the femoral tunnel in an inside-out manner [11] (width 8 mm and depth 35 mm) and fixed the hamstring with the Rigidfix (Mitek, Norwood, MA), passing the device via the anterolateral (AL) portal. The aim of this anatomical cadaver study is to evaluate the entrance points of cross-pins and the possible iatrogenic damages, during transverse fixation via anterolateral portal. The device we used is the Rigidfix (Mitek, Norwood, MA), designed to use two biodegradable pins for ACL femoral fixation. Femoral drilling and cross pins insertion were performed on 20 (10x2) fresh frozen cadaver knees. The risk of chondral damage, evaluated during the knees dissection, is high.

The aim was to study the independent risk factors of internal fixation failure in proximal femoral anti-rotation intramedullary nailing for intertrochanteric femur fracture, and to build a nomogram prediction model accordingly. Clinical data of patients with intertrochanteric femoral rotor fractures admitted to the First People's Hospital of Longquanyi District from January 2018 to January 2023 were retrospectively collected. The occurrence of spiral blade cut out, internal fixation breakage, peri-internal fixation fracture, hip internal rotation deformity, and fracture nonunion within 1 year after surgery were included in the internal fixation failure group, and the rest were included in the internal fixation success group.

As surgeons, we strive to recognize and correct any mistakes that may occur before completing an operation, and importantly, do our best to avoid irreversible mistakes. Over-resection of the femoral cam lesion in patients having hip arthroscopy for femoroacetabular impingement syndrome has been considered irreversible. While cam under-resection is a technical complication of femoroacetabular impingement surgery to be avoided, avoiding this at the expense of over-resection of the proximal femur is of great concern.

Geriatric femoral neck fractures are common orthopaedic injuries, which are associated with a high morbidity and mortality. Arthroplasty is the optimum treatment for many of these injuries, but debate exists regarding optimal surgical strategy. Multiple recent investigations have demonstrated strong superiority for cemented stems as compared with noncemented fixation with a decreased risk of periprosthetic fracture, shorter length of stay, lower cost, and decreased rate for revision surgery.
