The first enantioselective total syntheses of the beta-carboline alkaloids (-)-isochrysotricine (1) and (-)-isocyclocapitelline (2) are reported which confirm the absolute configuration of these natural products. Key steps are the copper-mediated S(N)2'-substitution of propargyl oxiranes 13/14 and the gold-catalyzed cycloisomerization of alpha-hydroxyallene 15, resulting in a highly efficient center-to-axis-to-center chirality transfer.

Active targeting of type 1 diabetes therapies to pancreatic beta cells using nanocarriers.

Diabetologia

Department of Chemical and Biological Engineering, Colorado School of Mines, Golden, CO, USA.

Type 1 diabetes is an autoimmune disease characterised by the destruction of pancreatic beta cells, resulting in lifelong insulin dependence. Although exogenous insulin can maintain glycaemic control, this approach does not protect residual or replacement pancreatic beta cells from immune-mediated death. Current therapeutics designed to protect functional beta cell mass or promote beta cell proliferation and regeneration can have off-target effects, resulting in higher dose requirements and adverse side effects.

Computer-assisted surgery is becoming essential in modern medicine to accurately plan, guide, and perform surgeries. Similarly, Digital Twin technology is expected to be instrumental in the future of surgery, owing to its capacity to virtually replicate patient-specific interventions whilst providing real-time updates to clinicians. This perspective introduces the term Digital Twin-Assisted Surgery and discusses its potential to improve surgical precision and outcome, along with key challenges for successful clinical translation.

Respiratory support strategies in neonatal transport in the UK and Ireland.

Eur J Pediatr

January 2025

Department of Women and Children's Health, School of Life Course Sciences, Faculty of Life Sciences and Medicine, King's College London, Denmark Hill, London, UK.

Unlabelled: Infants requiring interhospital transfer for a higher level of care in the neonatal period are at increased risk of adverse outcomes. Optimising respiratory management is an important priority. The aim of this survey was to investigate current respiratory support strategies in neonatal transport and identify opportunities for the optimisation of clinical care and future research.

Nucleic acid detection is considered the golden standard for diagnosing infectious diseases caused by various pathogens, including viruses, bacteria, and parasites. PCR and other amplification-based technologies are highly sensitive and specific, allowing for accurate detection and identification of low-level causative pathogens by targeting and amplifying their unique genetic segment (DNA or RNA). However, it is important to recognize that machinery-dependent diagnostic methods may only sometimes be available or practical in resource-limited settings, where direct implementation can be challenging.

Background: Professional standards and the Codes of Ethics from the International Council of Nurses and American Nurses Association are frameworks driving professional practice and are linked to positive patient and nursing outcomes.

Objective: To describe qualitative themes extracted from narrative responses on how nurses' personal values influence nursing care and converge with quantitative results assessing personal values, professional values, and the practice environment, a key component of nurse satisfaction and subsequent care quality.

Methods: A convenience sample of nurses across 19 sites within a large nonprofit healthcare system was surveyed.
