Actinomycotic conjunctivitis is usually unilateral and secondary to canaliculitis. We report an unusual case of bilateral actinomycotic blepharokeratoconjunctivitis in the absence of canaliculitis. Keratitis occurred without any preceding ocular trauma. Both eyes had remissions and relapses in response to the antibiotic-steroid drops. Diagnosis of A. israelii infection was made on the basis of Gram staining, culture and biochemical characteristics. Complete excision of the conjunctival ulcer along with penicillin therapy resulted in cure. This case highlights that microorganisms of the order Actinomycetales should be considered in the differential diagnosis if conjunctivitis or keratitis shows a waxing and waning course.

Background: Actinomycosis is a granulomatous infection produced by filamentous, gram-positive anaerobic bacteria. Due to its rarity, ambiguous symptoms, and resemblance to more frequent disorders, including cancer, Crohn's disease, and tuberculosis, it is a challenging disease to identify preoperatively.
Case Presentation: Our case was a case of a 36-year-old woman from the Oromia region's West Arsi zone, who presented with a 6-month history of snoring, dysphonia, and cough, as well as significant but unquantified weight loss, fatigue, and low-grade and intermittent fever, for which she had visited various health facilities with no noticeable improvements in her symptoms.

Case Summary: An 18-month-old male neutered Ragdoll cat presented with an 8 week history of progressive unilateral right-sided mucopurulent nasal discharge and exophthalmos. Magnetic resonance imaging revealed a heterogeneous right retrobulbar mass and bilateral nasal cavity disease. Filamentous structures seen on cytology of retrobulbar and nasal biopsies were mistakenly identified as filamentous fungal hyphae.
