Impact of Ga-FAPI positron emission tomography/computed tomography on staging and tumor management in patients with gastric cancer.

J Cancer Res Clin Oncol

Department of Medical Oncology, Cancer Center, West China Hospital, Sichuan University, 37 Guoxue Alley, Chengdu, Sichuan, 610041, China.

Purpose: To evaluate the added value of additional Ga-FAPI PET/CT following CT for primary staging, detection of postoperative recurrence, and management of gastric cancer patients.

Methods: We retrospectively included patients with gastric cancers who underwent contrast-enhanced computed tomography (ceCT), followed by Ga-FAPI PET/CT within 30 days. Ga-FAPI PET/CT was performed for initial staging or detection of postoperative recurrence.

Background: Postoperative complications may lower the quality of life of patients, consequently leading to a reduction in their overall survival (OS). In our previous investigations, we found that patients with gastric cancer (GC) with postoperative complications who underwent direct surgery had a significantly lower OS than patients without complications. We observed no significant difference in OS among patients who underwent neoadjuvant chemotherapy (NAC), regardless of complications.

Article Synopsis
  • Radical resection is still the main treatment option, but the overall 5-year survival rate remains below 35%, influenced by global variations in treatment approaches, resources, and patient conditions.
  • A comprehensive and multidisciplinary strategy is crucial for better management, with emerging methods like endoscopic resection for early-stage cancers and neoadjuvant therapy for advanced cases showing promise for improved survival and quality of life.

Surgical resection is the cornerstone of treatment for locally advanced gastric cancer (LAGC). Hence, downstaging of the tumor with neoadjuvant therapy is critical for R0 resection and prolongs the overall survival. Data from related studies are lacking, and the literature is scarce.

Mutations in doublecortin domain-containing protein 2 (DCDC2) lead to neonatal sclerosing cholangitis (NSC), and portal hypertension (PHTN). The objective of the study was to systematically evaluate PHTN, variceal bleeding, and outcomes of patients with DCDC2-related NSC. The study included children with homozygous or compound heterozygous variants in DCDC2.
