Background: The primary requirement for a capable patient health record (PHR) that can interact appropriately with the family medicine system and collect and share qualified data is a minimum data set (MDS) aligned with family medicine's functions and goals. The aim of this research was to determine the MDS for a blockchain-based PHR system that can effectively interact with family medicine providers and collect and share qualified data. This MDS is intended to be applicable to all members of the community covered by family medicine.

Background: Personal health records (PHRs) are consumer-centric tools designed to facilitate the tracking, management, and sharing of personal health information. PHR research has mainly been conducted in high-income countries rather than in low- and middle-income countries. Moreover, previous studies that proposed PHR design in low- and middle-income countries did not describe integration with other systems, or there was no stakeholder involvement in exploring PHR requirements.

Personal health records (PHRs) have been implemented around the world as a means to increase the safety, quality, and efficiency of health care. The Ministry of National Guard Health Affairs in the Kingdom of Saudi Arabia launched their PHR in 2018. Achieving predictive, precise, participatory, preventive, and personalized health (abbreviated as p-Health) requires comprehensive evaluations of an individual's conditions captured by various measurement technologies. Since the 1950s, analysis of care providers' and physicians' notes and measurement data by computers to improve healthcare delivery has been termed clinical informatics. Since the 2010s, wide adoptions of Electronic Health Records (EHRs) have greatly improved clinical informatics development with fast growing pervasive wearable technologies that continuously capture the human physiological profile in-clinic (EHRs) and out-of-clinic (PHRs or Personal Health Records) to bolster mobile health (mHealth).
