Introduction: Multiple sclerosis (MS) is the most common primary demyelinating disease affecting the central nervous system. In recent years the development of new drugs that have been shown to modify the natural history of MS have had a substantial impact on the treatment of the disease.

Aims: To harmonise and integrate the evidence available on optimising the treatment of patients with MS.

Development: In order to fulfil our main aim, a group of experts from different Latin American countries drew up a list of statements related to the use of immunomodulatory agents in the different clinical forms of the disease and the strategies that should be considered in cases in which the therapeutic response was suboptimal. Each of the participants used a structured scale to express the extent to which he or she agreed or disagreed, and a consensus was considered to have been reached when acceptance of each of the statements was equal to or higher than 80%.

Conclusions: These recommendations will provide neurologists with the tools needed to make decisions that optimise the treatment of MS patients.

Background: Metastasis of non-gastrointestinal (non-GI) cancers to the upper GI tract is a rare occurrence, with limited cases reported in the literature. Recognising this type of metastasis is crucial, as it presents unique diagnostic and therapeutic challenges. This case series adds to the literature by discussing seven rare cases of non-GI cancer metastasising to the upper GI tract, emphasising the complications and clinical manifestations.

Prescribing errors are a source of preventable harm in healthcare, which may be mitigated using Electronic Prescribing (EP) systems. Anyone who routinely prescribes medication could benefit from digitally assisted automated checks to identify whether a prescription should potentially not be allowed (e.g.

Ulcerative colitis, a chronic inflammatory condition of the colon, requires precise and targeted treatment, and polysaccharides, with their pH responsiveness and biodegradability, offer an innovative approach for colon-specific drug delivery. This study aims to develop a highly precise drug delivery system with enhanced therapeutic and targeting efficiency for ulcerative colitis, focusing on the preparation, optimisation, and evaluation of dual cross-linked mesalamine-loaded sericin-pectin (DSPs) micro-beads. These beads utilise the pH-responsive and microflora biodegradability properties of polysaccharides for targeted colon delivery, employing the Response Surface Methodology.

January 2025

LKS: Rheumatologist, MBChB, FRACP, PhD, Department of Rheumatology, Immunology and Allergy, Rheumatology Registrar Te Whatu Ora Waitaha, New Zealand and Department of Medicine, University of Otago Christchurch, Christchurch, New Zealand.

Objective: Despite effective treatment, gout is poorly managed. The aim of this study was to determine rates of serum urate (SU) testing and allopurinol dose adjustment in patients on allopurinol admitted to Christchurch based hospitals.

Methods: The hospital electronic prescribing and administration (ePA) system was used to identify patients on allopurinol during hospital admissions from March 2016-March 2023.

Introduction: T lymphocyte collection is essential for CAR T-cell engineering in refractory hematologic malignancies but needs to be optimised. No guidelines have been established for predicting the amount of T lymphocytes to be collected. The quantity of lymphocytes and especially T cells collected depends on the pre-cytapheresis lymphocyte blood level (pcLBL) and the number of blood volumes (BVs) processed.
