AI Article Synopsis

  • The Resource Based Relative Value System (RBRVS) was created to organize the Medicare reimbursement process over fifteen years ago.
  • The system uses a complex formula to assign value to various medical procedures based on multiple components that are calculated differently.
  • The article explores how these relative values are developed, managed, and how their relationships impact the overall reimbursement system.

Article Abstract

The Resource Based Relative Value System (RBRVS) was established over fifteen years ago in an attempt to bring order to the Medicare reimbursement system. The RBRVS relies on a complicated formula to determine how much each procedure is worth. This article uses an unusual approach to describe the components of the RBRVS equation, how those relative values are developed and managed, and how they relate to each other, even though each one is calculated differently.

Optimizing Physician Payment for a Single-Payer Healthcare System.

Int J Soc Determinants Health Health Serv

IHPS, UCSF, Berkeley, California, USA.

Current forms of payment of independent physicians in U.S. health care may incentivize more care (fee-for-service) or less care (capitation), be inequitable across specialties (resource-based relative value scale [RBRVS]), and distract from clinical care (value-based payments [VBP]).

Objectives: To carry out a comparative analysis between 3 different workload measurement systems in surgical pathology: the Resource-Based Relative Value Scale (RBRVS), the Level 4 Equivalent (L4E), and the Automatable Activity-Based Approach to Complexity Unit Scoring (AABACUS). The RBRVS is one of the most widely used systems in terms of attempting to measure workload, whereas it has been proposed as a means of costing (and thus setting reimbursement rates) of surgical pathology services in Greece, despite being widely criticized for its inaccurate design.

Methods: Surgical pathology workload for 1 representative month at Evaggelismos General Hospital was assessed using both the RBRVS and the 2 newer methods.

Measuring work complexity for acute care services.

Int J Health Plann Manage

November 2021

Decision Sciences Institute, Fuzhou University, Fuzhou, China.

Case weights capture the resource cost by diagnosis-related group (DRG) but may not fully reflect the complexity of the clinical services provided. This study describes the use of a work complexity index (WCI), for assessing acute care services focusing on those provided by physicians in healthcare systems. The services are classified using relative value units (RVUs) and their point value assigned using the resource-based relative value scale.

Purpose: The Resource-based Relative Value Scale (RBRVS) designates quantitative values to clinical health care services to identify the relative work and cost of these services. Each clinical service translates to relative value units (RVUs). Clinical RVUs describe clinician work effort to guide employed clinician salaries in academic medical centers.
