A nine-year-old boy fell on his outstretched hand and was diagnosed with simultaneous fractures of the scaphoid and the capitate. The patient was treated conservatively with a cast for two months. Fracture of the capitate healed, however, fracture of the scaphoid resulted in non-union. We treated scaphoid non-union with an iliac bone graft and internal fixation. Three years after surgery, the patient had neither complaints nor complications and union of the scaphoid was confirmed on the radiographs.

New surgical techniques for the treatment of scaphoid non-union, developed in the last two decades, now enable a healing rate of 80-90%. However, no consensus exists for the surgical treatment of non-union. On the other hand, regenerative medicine techniques have enriched the therapeutic armamentarium for non-union, especially in the lower limbs, with the use of autologous concentrated bone marrow injection using autologous osteogenic precursors to create a favorable microenvironment for bone healing.

Introduction: Four-corner fusion has long been the preferred treatment for stages II and III of scaphoid nonunion advanced collapse with intact radiolunate articulation. Three corner fusions were then proposed as a more limited procedure with improved ulnar deviation through triquetrum excision. However, we believe triquetrum preservation would decrease the radiolunate contact pressure without affecting the ulnar deviation range.
