In breast-conservative therapy (BCT), it is difficult to repair defects in the inferior portion of the breast. We performed an immediate reconstruction using an inframammary adipofascial flap after breast conservation in 4 patients with breast cancer in the inferior portion of their breasts and evaluated the usefulness of the procedure. A skin incision is made at the inframammary line. Immediately following BCT, a tongue-shaped flap of the fat and the anterior sheath of the rectus abdominis muscle are pulled up and inserted to reconstruct the breast mound. The cosmetic outcomes of all patients were good, and this surgical procedure can be easily performed by general surgeons without the need of assistance by plastic surgeons. We believe this to be a useful surgical modality for the reconstruction of BCT in the inferior portion breast cancer.

Increased intolerance of uncertainty (IU), or distress felt when encountering situations with unknown outcomes, occurs transdiagnostically across various forms of psychopathology and is targeted in therapeutic intervention. Increased intolerance of uncertainty shows overlap with symptoms of internalizing disorders, such as depression and anxiety, including negative affect and anxious apprehension (worry). While neuroanatomical correlates of IU have been reported, previous investigations have not disentangled the specific neural substrates of IU above and beyond any overlapping relationships with aspects of internalizing psychopathology.

Purpose: To determine the clinical effects of urethral suspension-assisted urethral anastomosis on complex long-segment posterior urethral stricture and describe the technical aspects of this procedure.

Materials And Methods: The clinical data for 24 patients who underwent urethral suspension-assisted urethral anastomosis for complex long-segment posterior urethral stricture between March 2021 and March 2024 were retrospectively analyzed. The surgical procedure comprises the following four steps: creation of an inverted Y-shaped incision in the perineum; mobilization of the urethra up to the penile-scrotal junction followed by dissection and separation of the septum of the corpus cavernosum; separation of the inferior pubic symphysis, excising a portion of the inferior pubic symphysis bone tissue and thoroughly clearing the scar tissue surrounding the proximal urethra; and suturing and suspension of the proximal urethra and surrounding tissues at the 2, 5, 7, and 10 o'clock positions, ensuring complete exposure of the proximal urethral mucosa and tension-free anastomosis between the proximal and distal urethra.
