Trauma-induced dystonia and camptocormia in a child. | LitMetric

A 14-year-old male sustained neck trauma during a fight at school. Torticollis developed immediately afterwards, followed by axial dystonia and camptocormia. Thorough evaluation for etiology or background disease, including psychiatric examination, was negative except for the recent trauma. Antidystonia medications, administered after significant worsening of the symptoms, led to improvement. Dystonia and camptocormia resulting from trauma are rare presentations in childhood and adolescence.

Dystonia is a movement disorder characterized by sustained or intermittent involuntary muscle contractions, which is also seen in an advanced stage of Parkinson's disease (PD) as camptocormia, torticollis, and Pisa syndrome. Istradefylline, an adenosine A receptor antagonist, can be used for the treatment of PD to reduce 'off'-time period, and several clinical studies demonstrated the improvement of camptocormia, which have many similar features to dopa-responsive/non-responsive dystonia. Many animal models of dystonia showed that adenosine A receptor colocalized with dopamine D2 positive spiny projection neurons in indirect pathway of basal ganglia circuit, and also in the cholinergic interneurons that affects the balance of indirect and direct pathway of basal ganglia.

Many studies have shown that botulinum toxin (BoNT) can be an option to treat motor and non-motor symptoms in Parkinson's disease (PD) and parkinsonian syndromes. The advantages of BoNT compared to oral medications include localized action and low incidence of systemic side effects, which is important in treating neurodegenerative disease. Motor symptoms that can be treated with BoNT include blepharospasm, apraxia of eyelid opening, tremor, cervical dystonia and limb dystonia.
