Background: Entropy quantifies the level of disorder within a system. Low entropy reflects increased rigidity of homeostatic feedback systems possibly reflecting failure of protective physiological mechanisms like cerebral autoregulation. In traumatic brain injury (TBI), low entropy of heart rate and intracranial pressure (ICP) predict unfavorable outcome.

Objectives: The purpose of this study was to determine whether our new thinking guidance named OPACCUS (oxygen metabolism, perfusion, arterial tension, cardiac output, systemic congestion, unregulated host response and search for inciting illness event) with 7 questions you need to ask before shock therapy and evidences provided by critical ultrasound considering hemodynamics, the unregulated host response and inciting illness event would improve mortality in shock patients.
Design: A multicenter, prospective, observational cohort study.
Setting: Intensive care units of 20 hospitals in Southwest China.

Disseminated venous thromboembolism (VTE) occurs commonly in cancer patients, who tend to have contraindications to systemic thrombolysis and require cancer surgery. Such clinical scenarios are often challenging to manage. In this case report, we illustrate an innovative, single procedural approach in such a patient to remove extensive VTE, improve symptoms, prevent hemodynamic decompensation, and allow for a minimal level of anticoagulation such that necessary cancer surgery can proceed safely.

Objectives: The pathophysiological mechanisms of status epilepticus (SE) underlying potential brain injury remain largely unclear. This study aims to employ functional near-infrared spectroscopy (fNIRS) combined with video-electroencephalography (vEEG) to monitor brain hemodynamics continuously and non-invasively in critically ill adult patients experiencing electrographic SE. Our primary focus is to investigate neurovascular coupling and cerebrovascular changes associated with seizures, particularly during recurring and/or prolonged episodes.
