We describe a case in which the patient developed profound, intermittent hypoxemia after implantation of a permanent pacemaker and subsequently after revision of its leads. Cardiac catheterization demonstrated right-to-left shunting at the level of the atria in the presence of a patent foramen ovale that required closure, resulting in the resolution of symptoms. Our report highlights the fact that the presence of a patent foramen ovale with intermittent right-to-left shunting should be considered in a differential diagnosis of hypoxemia after implantation of heart rhythm devices.

Introduction: Obstructive sleep apnoea (OSA) is characterised by blood oxygen desaturations and sleep disruptions manifesting undesirable consequences. Existing treatments including oral appliances, positive airway pressure (PAP) therapy and surgically altering the anatomy of the pharynx have drawbacks including poor long-term adherence or often involving irreversible, invasive procedures. Bilateral hypoglossal nerve stimulation (HNS) is a new treatment for managing OSA, and this study is intended to determine whether an HNS system is a safe and effective treatment option for adults with OSA.

Reactive oxygen species (ROS) is promising in cancer therapy by accelerating tumor cell death, whose therapeutic efficacy, however, is greatly limited by the hypoxia in the tumor microenvironment (TME) and the antioxidant defense. Amplification of oxidative stress has been successfully employed for tumor therapy, but the interactions between cancer cells and the other factors of TME usually lead to inadequate tumor treatments. To tackle this issue, we develop a pH/redox dual-responsive nanomedicine based on the remodeling of cancer-associated fibroblasts (CAFs) for multi-pronged amplification of ROS (ZnPP@FQOS).

Mesenchymal stem cells (MSC) possess unique immunomodulatory properties and have enormous potential in the treatment of graft-versus-host disease (GVHD). However, the low implantation and survival rates of MSC in vivo, coupled with their weak immunosuppressive functions, have resulted in unstable clinical efficacy in the treatment of GVHD. Preconditioning of MSC with hypoxia, active molecules and gene modification can enhance the function of MSC and improve the implantation rate, survival rate and therapeutic effect of MSC.

Bone fractures are associated with hypoxia, but no longitudinal studies of perfusion measurements in human patients have been reported despite the clinical and research potential. In this longitudinal observational cohort study, the near-infrared spectroscopy (NIRS) device PortaMon was used to assess oxy-(OHb), deoxy-(HHb) and total (tHb) haemoglobin, as well as the differences between OHb and HHb (Hb) and the tissue saturation index (TSI) at three different depths in the fracture gap. Linear mixed effect models were fitted to analyse time effects.
