This article describes an unusual case of internal resorption in primary molars followed by apposition of calcified deposits. Expanded borders of the coronal pulp chambers of all four first primary molars were first observed on a routine recall examination at age 7 years 7 months. Exposure of the process to the surrounding bone linked the bone to the calcified tissue formed in the pulp and caused ankylosis and infraocclusion of the affected teeth. The internal resorption and remodelling process in this case were of an unknown aetiology. Two possible explanations are suggested: (1) pulp response to a mild but lasting stimulation created by bruxism, and (2) pulp response to a short but forceful closure of the mouth due to a chin injury at age 3. Spontaneous arrest of internal resorption processes in primary molars may be considered as a possible outcome if the process does not have an inflammatory background.
