Irregular sleep-wake rhythm disorder (ISWRD) is an intrinsic circadian rhythm disorder caused by loss of the brain's circadian regulation, through changes of the input and/or output to the suprachiasmatic nucleus (SCN), or of the SCN itself. Although there are limited prevalence data for this rare disease, ISWRD is associated with neurodegenerative disorders, including the Alzheimer disease (AD) and the Parkinson disease (PD), which will become increasingly prevalent in an aging population. It additionally presents in childhood developmental disorders, psychiatric disorders, and traumatic brain injury (TBI).

This study aimed to examine the sleep parameters and sleep/wake regularity of a cohort of student-athletes who start training between 06:30 and 07:00. Twenty-one male Rugby Union players, aged 21 ± 2 years and competing at a national level, were assessed using actigraphy over two weeks, and the Athlete Sleep Screening Questionnaire (ASSQ). Sleep/wake regularity was calculated using the Sleep Regularity Index (SRI).

Circadian Misalignment Impacts Cardiac Autonomic Modulation in Adolescence.

Sleep

January 2025

Sleep Research & Treatment Center, Department of Psychiatry & Behavioral Health, Penn State University, College of Medicine, Hershey PA, USA.

Study Objectives: Although heart rate variability (HRV), a marker of cardiac autonomic modulation (CAM), is known to predict cardiovascular morbidity, the circadian timing of sleep (CTS) is also involved in autonomic modulation. We examined whether circadian misalignment is associated with blunted HRV in adolescents as a function of entrainment to school or on-breaks.

Methods: We evaluated 360 subjects from the Penn State Child Cohort (median 16y) who had at least 3-night at-home actigraphy (ACT), in-lab 9-h polysomnography (PSG) and 24-h Holter-monitoring heart rate variability (HRV) data.

Context: Sleep health is multidimensional. While studies have shown associations between certain sleep dimensions and health in type 1 diabetes (T1D), global sleep health has rarely been considered.

Objective: To examine the associations between individual sleep dimensions and multidimensional sleep health (MSH) on glycemic control and self-reported outcomes in T1D.

Background: Digital wearable devices, worn on or close to the body, have potential for passively detecting mental and physical health symptoms among people with severe mental illness (SMI); however, the roles of consumer-grade devices are not well understood.

Objective: This study aims to examine the utility of data from consumer-grade, digital, wearable devices (including smartphones or wrist-worn devices) for remotely monitoring or predicting changes in mental or physical health among adults with schizophrenia or bipolar disorder. Studies were included that passively collected physiological data (including sleep duration, heart rate, sleep and wake patterns, or physical activity) for at least 3 days.
