A 75-year-old man with a large pituitary adenoma presented with general muscular weakness, including difficulty in rising in the morning and forceless defecation and urination, and muscular pain, numbness and atrophy in the shoulder, neck and thigh. Testosterone replacement and subsequent resection of the pituitary tumor resulted in resolution of the symptoms. The value of pituitary imaging tests for men with hypogonadotropic hypogonadism is discussed.

Background: Giant pituitary neuroendocrine tumor (GPitNET) are challenging tumors with low rates of gross total resection (GTR) and high morbidity. Previously reported machine-learning (ML) models for prediction of pituitary neuroendocrine tumor extent of resection (EOR) using preoperative imaging included a heterogenous dataset of functional and non-functional pituitary neuroendocrine tumors of various sizes leading to variability in results.
Objective: The aim of this pilot study is to construct a ML model based on the multi-dimensional geometry of tumor to accurately predict the EOR of non-functioning GPitNET.

Non-functioning pituitary adenomas (NFPAs) are hormonally inactive benign tumors, usually diagnosed as macro-adenoma. The aim of our research was to analyze the clinical and hormonal characteristics of NFPAs using Knosp and revised Knosp classifications. Background: Temozolomide (TMZ), a non-classical alkylating agent, possesses lipophilic properties that allow it to cross the blood-brain barrier, making it active within the central nervous system. Furthermore, the adverse reactions of the TMZ are relatively mild, which is why it is currently recommended as a first-line chemotherapy drug for refractory pituitary adenomas (RPAs) and pituitary carcinomas (PCs).
Summary: Systematic evaluations indicate a radiological response rate of 41% and a hormonal response rate of 53%, underscoring TMZ clinical efficacy, particularly when combined with radiotherapy.
