An unusual case of purely cystic bilateral ancient schwannomas of the orbit is reported. Complete ophthalmologic examination and CT was performed. Diagnosis was established by excisional biopsy of both tumors, including clinicopathologic study. Histologic examination showed a bilateral purely cystic ancient schwannoma. This case underlines the importance of considering neural tumors, including schwannomas, in the differential diagnosis of both cystic and bilateral orbital tumors. As far as we know, no other case has been previously reported.

Schwannoma is a rare mass that is benign. It develops from the Schwann cells found in the peripheral nerves. Schwannomas consists of spindle cells exhibiting two unique histological patterns, which may be combined: Antoni type A and type B.

Introduction: Schwannomas are benign solitary, slow-growing, encapsulated, asymptomatic tumors arising from Schwann's sheath of nervous fibers.
Objectives: The current paper presents all the cases diagnosed with different types of schwannomas in the head and neck region between January 2009 and December 2023 in the Western part of Romania. In this period, ten cases of schwannoma were identified.

Key Clinical Message: Benign ancient retroperitoneal schwannomas (BARS) exhibit abdominal masses and flank pain to incidental findings at more advanced stages. Histopathological and immunohistochemical analysis is essential for confirmation of benign nature. Our patient was misdiagnosed as ureteric colic, highlighting the need to consider BARS in differential diagnosis to prevent complications like hydronephrosis.
