Diagnosis of primary aldosteronism has increased dramatically in recent years with the wider availability of aldosterone and plasma renin activity measurements. The authors present the case of a patient with resistant hypertension and severe hypokalemia that evaded diagnosis until complications from hypokalemia occurred.
