Platelet satellitism is an infrequent phenomenon of platelet adhesion to leucocytes. Twelve-month-old patient was hospitalized because of bronchopneumonia. In peripheral blood smear prepared from EDTA-anticoagulated blood sample taken from this patient platelet satellitism was observed. As time went by, a gradual, small decrease of platelet count was observed. Cytometric analysis showed the following proportions of platelet aggregates: with granulocytes--88,7%, with monocytes--5,0%, with lymphocytes--17,0%. In spite of using modern hematological analyzers still microscopic assessment of peripheral blood sample is necessary. Microscopic examination of blood smear is necessary not only to assess erythrocytes and leucocytes systems but also blood platelets.

Platelet satellitism refers to the rosetting of the platelets around white blood cells, mostly neutrophils that could lead to spuriously low platelet counts on automated analyzers. The phenomenon has usually been described in EDTA processed blood samples. We describe the clinical course of a patient with immune thrombocytopenia with platelet satellitism in both EDTA as well as non EDTA processed blood samples.

Platelet cold agglutination (PCA) is a rare in-vitro phenomenon caused by Immunoglobulin M (IgM) autoantibodies, which results in Ethylenediaminetetraacetic Acid (EDTA) independent pseudo thrombocytopenia (PTCP). Platelet satellitism is uncommon phenomenon characterized by formation of platelet rosette around polymorphonuclear leucocytes in blood smear prepared from Ethylenediaminetetraacetic acid anticoagulated blood. This phenomenon may cause reporting of spurious thrombocytopenia unless proper examination of blood smears. Here we describe a case of platelet satellitism in a 29 year old female which has been incidentally found on evaluation of eosinophilia in a bronchial asthma patient.
