Cytomegalovirus-associated supraglottic mass in a patient on immunosuppressive therapy.

S Afr Med J

June 2021

Division of Infectious Diseases, Department of Medicine, School of Clinical Medicine, University of the Witswatersrand, Johannesburg, South Africa.

A 33-year-old woman on chronic immunosuppressive treatment for rheumatoid arthritis with a history of inhaled methamphetamine use presented with respiratory failure requiring mechanical ventilation for a prolonged period. After being given plasma exchange, pulses of methylprednisolone and a dose of cyclosporine for suspected ANCA (anti-neutrophilic cytoplasmic autoantibodies) vasculitis, she developed an obstructive supraglottic laryngeal mass that required a tracheostomy to bypass. Biopsy findings revealed the mass to be an inflammatory pseudomass secondary to cytomegalovirus (CMV).

Objective: We sought to characterize the penetrance, clinical features, and best treatment options in 133 CTLA4 mutation carriers.

We present a case of cytomegalovirus (CMV)-induced pseudotumor of the gastric antrum. Although affliction of the entire gastrointestinal tract with CMV has been described, localization to the stomach and especially the gastric antrum is rare. Kaposi's sarcoma and non-Hodgkin's lymphoma are recognized causes of bowel thickening and obstruction in patients with AIDS, but CMV is an extremely rare cause, with only four cases of CMV-induced pseudotumor reported in the English literature.

Cytomegalovirus (CMV)-associated gastrointestinal masses have been reported in human acquired immune deficiency syndrome patients. This is the first report on CMV-associated gastrointestinal masses in simian immunodeficiency virus (SIV)-infected macaques. Two SIV-infected macaques presented at necropsy with multiple nodular or umbilicated masses within the gastrointestinal tract.
