The present report describes a case of osteoid osteoma of the trapezoid. It'is an unusual localisation. Synovitis was the first clinical symptom. Tomograms allowed diagnosis and guided the treatment. Radical excision prevented recurrence.

PURPOSE OF THE STUDY Osteoid osteoma (OO) of the upper extremity is a rare pathology representing a major diagnostic challenge. The patients are often times incorrectly treated due to a misdiagnosis and therefore the final management of symptoms is delayed by several months. Osteoid osteoma is a benign, bone-forming tumor that rarely involves the carpal bones. We report a case of osteoid osteoma of the trapezoid carpal bone with extension to the adjacent second metacarpal bone. Chronic wrist pain and local tenderness were the major clinical signs and symptoms.

The occurrence of osteoid osteoma of the carpus is rare, particularly of the trapezoid. We present a case of a ten-year-old girl with osteoid osteoma of the trapezoid in whom surgical excision of the lesion was successful. Useful information for the diagnosis of the lesion was mainly provided by dynamic magnetic resonance imaging.
