A derivative of pyrimido[4,5-d]pyrimidine-2,4,5,7-(1H,3H,6H,8H)-tetraone (PPT), which may form various keto-enol tautomers suitable for forming base pairs with all natural bases, and is thus expected to serve as a universal base, was synthesized. The ability of PPT to form base pairs with natural bases was evaluated by UV analysis.

The genetic code is a universally conserved mechanism that translates genetic information into proteins, consisting of 64 codons formed by four nucleotide bases. With a few exceptions, the genetic code universally encodes 20 canonical amino acids (AAs) and three stop signals, with many AAs represented by multiple codons. Genetic engineering has expanded this system through approaches like codon reassignment and synthetic base pair introduction, allowing for the incorporation of noncanonical AAs (ncAAs) into proteins, known as genetic code expansion (GCE).

Background: Sleep is substantial issue for hospital inpatients and can negatively affect healing and recovery. There is a good evidence-base for interventions which can improve sleep, however currently they are not being implemented into NHS practice. Objective: Estimate the budget impact of funding a standardized education and exercise therapy program (GLA:D®) for people with hip and knee OA waiting for total joint replacement (TJR) consultation in a universal publicly insured healthcare system in Canada.

Methods: We built a budget impact analysis (BIA) model to estimate the annual cost of providing GLA:D® program to people waiting for TJR consultation and then forecasted a three-year budget cycle. The base case assumes 40% attend GLA:D®, 11% avoid surgery, uniform care delivery, training costs are incurred separately, and the healthcare system has enough trained staff to meet demand.
