Randomization as a basis for inference in noninferiority trials.

Noninferiority testing in clinical trials is commonly understood in a Neyman-Pearson framework, and has been discussed in a Bayesian framework as well. In this paper, we discuss noninferiority testing in a Fisherian framework, in which the only assumption necessary for inference is the assumption of randomization of treatments to study subjects. Randomization plays an important role in not only the design but also the analysis of clinical trials, no matter the underlying inferential field. The ability to utilize permutation tests depends on assumptions around exchangeability, and we discuss the possible uses of permutation tests in active control noninferiority analyses. The other practical implications of this paper are admittedly minor but lead to better understanding of the historical and philosophical development of active control noninferiority testing. The conclusion may also frame discussion of other complicated issues in noninferiority testing, such as the role of an intention to treat analysis.

: Thanks to the evolution of laboratory medicine, point-of-care testing (POCT) for troponin levels in the blood (hs-cTn) has been greatly improved in order to quickly diagnose acute myocardial infarction (AMI) with an accuracy similar to standard laboratory tests. Background: There is increasing recognition that the interpretation of active-controlled HIV prevention trials should consider the counterfactual placebo HIV incidence rate, that is, the rate that would have been observed if the trial had included a placebo control arm. The PrEPVacc HIV vaccine and pre-exposure prophylaxis trial (NCT04066881) incorporated a pre-trial registration cohort partly for this purpose. In this article, we describe our attempts to model the counterfactual placebo HIV incidence rate from the registration cohort.

Objective: This study investigates whether urinary cortisol (UC) and UC-to-creatinine ratio (UCCR) perform better than basal serum cortisol (BSC) in identifying dogs with hypoadrenocorticism (HA).

Methods: A retrospective, multicenter study with 120 client-owned dogs included: 20 with HA, 42 healthy, and 60 with diseases mimicking HA. The UC and UCCR were determined on urine samples using a chemiluminescent enzyme immunoassay.

Cardiac rehabilitation (CR) is a cornerstone of heart disease (HD) management, enhancing functional capacity and quality of life. Hybrid cardiac rehabilitation (hCR), combining supervised center-based sessions with synchronous, real-time telerehabilitation at home, offers an alternative to conventional CR to overcome logistical barriers such as facility limitations, distance, and pandemic-related disruptions. This randomized controlled trial evaluated the noninferiority of hCR compared to standard CR in improving functional capacity in patients with chronic heart disease, including those with stable coronary artery disease.
