Afferent loop obstruction is a relatively rare but significant complication of Billroth II gastrojejunostomy. We report the imaging findings in a patient in whom obstruction presented acutely and was due to the presence of an enterolith. CT showed dilatation of both the main pancreatic duct and the biliary ducts, and a markedly dilated afferent loop within which a 5-cm mass was present. The lesion had a heterogeneous, laminated appearance and did not show any contrast enhancement. Edema of fatty tissues surrounding the pancreatic tail, which extended to the left pararenal spaces, a small amount of free peritoneal fluid surrounding the spleen, and an aneurysm of the splenic artery of about 3 cm were also present. The diagnosis of afferent loop obstruction has to be considered in patients with previous Billroth II gastrojejunostomy who present with acute abdominal pain and laboratory findings indicating pancreatitis. Although rarely, an enterolith can be the cause of obstruction. CT allows to establish the diagnosis.

Neuromodulation comes into focus as a non-pharmacological therapy with the vagus nerve as modulation target. The auricular vagus nerve stimulation (aVNS) has emerged to treat chronic diseases while re-establishing the sympathovagal balance and activating parasympathetic anti-inflammatory pathways. aVNS leads still to over and under-stimulation and is limited in therapeutic efficiency.

Background: Juxtaglomerular (JG) cells are sensors that control blood pressure and fluid-electrolyte homeostasis. In response to a decrease in perfusion pressure or changes in the composition and/or volume of the extracellular fluid, JG cells release renin, which initiates an enzymatic cascade that culminates in the production of angiotensin II (Ang II), a potent vasoconstrictor that restores blood pressure and fluid homeostasis. In turn, Ang II exerts a negative feedback on renin release, thus preventing excess circulating renin and the development of hypertension.

Computational modelling of cardiac control following myocardial infarction using an in silico patient cohort.

Loss of cardiac physiological function following myocardial infarction (MI) is accompanied by neural adaptations in the baroreflex that are compensatory in the short term, but then become associated with long-term disease progression. One marker of these adaptations is decreased baroreflex sensitivity, a strong predictor of post-MI mortality. The relative contributions of cardiac remodelling and neural adaptation in the sensory, central brainstem and peripheral ganglionic loci to baroreflex sensitivity changes remain underexplored.

Neonatal pain is a significant clinical issue but the mechanisms by which pain is produced early in life are poorly understood. Our recent work has linked the transcription factor serum response factor downstream of local growth hormone (GH) signaling to incision-related hypersensitivity in neonates. However, it remains unclear if similar mechanisms contribute to inflammatory pain in neonates.

Endoscopic or fluoroscopic guided naso-enteric placement for stenting and decompression has been used in mechanical enteric limb obstruction after gastrectomy or gastric bypass surgery. However, the use of double naso-enteric tube for treatment of multiple enteric limbs obstruction has not been described to date. We present a 61-year-old female with afferent limb syndrome with concomitant efferent limb obstruction which caused by kinking of anastomosis after loop gastrojejunostomy for benign gastric outlet obstruction.
