Indole alkaloids from Cephalanceropsis gracilis.

Purification of CHCl3 and EtOAc solubles of the MeOH extract of Cephalanceropsis gracilis afforded seven new indole alkaloids, cephalinones A (1), B (2), C (3), and D (4) and cephalandoles A (5), B (6), and C (7), besides eight known compounds. The structures of the new compounds were determined by spectroscopic analysis. All 15 indole alkaloids were evaluated for their cytotoxic effects on MCF-7, NCI-H460, and SF-268 cell lines by the MTT method. Only cephalinone-F (6) showed significant cytotoxicity.

Ethnopharmacological Relevance: Zuojin Pill (ZJP) is a traditional Chinese medicine (TCM) formula composed of Coptidis Rhizoma and Euodiae Fructus in a ratio of 6:1 (w/w), which has been widely used for treating gastrointestinal disorders, especially stomach heat syndrome (SHS). However, the active alkaloids in ZJP showed low plasma exposure in rats following oral administration, which failed to explain their potent pharmacological effects, thereby limiting further mechanism studies.

Aim Of The Study: This study aimed to investigate the in vivo exposure and tissue distribution propensities of the active alkaloids in normal and SHS rats following oral administration of ZJP.

Treatment of Burkitt Lymphoma/Leukemia (BL/L) in adults has evolved from the use of pediatric inspired regimens (CODOX-M/IVAC, hyper-CVAD, GMALL) to the use of lower intensity EPOCH regimens. Here, we present the case of a young man active-duty service member diagnosed with stage IE penile diffuse large B-cell lymphoma (DLBCL), germinal centre B-cell (GCB) subtype. Primary penile DLBCL is an exceptionally rare condition, with fewer than 30 cases reported in the medical literature. Notably, this patient is significantly younger than the typical demographic, as extranodal DLBCL affecting the penis usually occurs in elderly men, with a median age of 69 years at diagnosis.
