Lasthénie de Ferjol syndrome is a very rare psychiatric illness that occurs mainly in women. It is characterized by severe recurrent iron-deficiency anemia caused be repeated episodes self-induced blood-letting. We report the case of a young homosexual male repeatedly admitted to various hospitals for severe hypochromic anemia. We discovered that the anemia was indeed due to psychotic self-provoked hemorrhages. Based on this experience and a review of the few cases reported in the literature, we discuss issues of early diagnosis, management and treatment of Lasthénie de Ferjol patients.

In this paper, we introduce an adolescent suffering from a rare form of self-mutilation, "self-bloodletting", and discuss dynamics of this maladaptive behavior. Deliberate bloodletting is a rare, but not unheard of, form of self-mutilation associated with eating and personality disorders. Although there are already several case reports in the adult psychiatry literature regarding the clinical features of Lasthenie de Ferjol Syndrome or factitious anemia, this is the first reported adolescent case of self-bloodletting.

In our paper, we present the fascinating story of Lasthénie de Ferjol syndrome. A rare self-induced iron deficiency anaemia caused by surreptitious blood-letting. The French haematologist Jean Bernard first described the syndrome and named it after the heroine of Barbey d' Aurevilly's novel The Story without a Name.
