Monoclonal antibodies to keratins No. 8 and 17 specific for lining epithelium and myoepithelium of the mammary gland, respectively, as well as to basement membrane laminin, entactin, collagen type IV and heparan sulfate proteoglycan were used to the immunohistochemical analysis of 77 benign and malignant human breast lesions and that of 38 cases in which an intraoperative biopsy diagnosis was difficult. Morphologically similar benign and malignant proliferations were distinguished by keratin expression. In benign lesions both keratins were present, while in malignant ones only keratin No. 8 was expressed. Basement membranes associated with a myoepithelial layer were intact in benign lesions and in situ structures, but they were absent around the vast majority of invasive tumor foci. Basement membrane loss was important in differential diagnosis of benign sclerosing adenosis and cystadenopapilloma from invasive tubular and papillary carcinoma, respectively. Diagnosis of microinvasion in ductal and lobular carcinoma was much easier when combination of antibodies to keratins and basement membrane proteins was used.

Objectives: External quality assessment (EQA) programs play a pivotal role in harmonizing laboratory practices, offering users a benchmark system to evaluate their own performance and identify areas requiring improvement. The objective of this study was to go through and analyze the UK NEQAS "Immunology, Immunochemistry and Allergy" EQA reports between 2012 and 2021 to assess the overall level of harmonization in autoimmune diagnostics and identify areas requiring improvement for future actions.
Methods: The EQA programs reviewed included anti-nuclear (ANA), anti-dsDNA, anti-centromere, anti-extractable nuclear antigen (ENA), anti-phospholipids, anti-neutrophil cytoplasm (ANCA), anti-proteinase 3 (PR3), anti-myeloperoxidase (MPO), anti-glomerular basement membrane (GBM), rheumatoid factor (RF), anti-citrullinated protein antibodies (ACPA), mitochondrial (AMA), liver-kidney-microsomal (LKM), smooth muscle (ASMA), APCA, and celiac disease antibodies.

In glomerulopathies, endothelial dysfunction and the presence of histological vascular lesions such as thrombotic microangiopathy, arteriolar hyalinosis, and arteriosclerosis are related to a severe clinical course and worse renal prognosis. The endothelial cell, which naturally has anti-inflammatory and anti-thrombotic regulatory mechanisms, is particularly susceptible to damage caused by various etiologies and can become dysfunctional due to direct/indirect injury or a deficiency of protective factors. In addition, endothelial regulation and protection involve participation of the complement system, factors related to angiogenesis, the renin-angiotensin system (RAS), endothelin, the glycocalyx, the coagulation cascade, interaction between these pathways, interactions between glomerular structures (the endothelium, mesangium, podocyte, and basement membrane) and interstitial structures (tubules, arterioles and small vessels).
