Different theoretical frameworks have been invoked to guide the study of virus evolution. Three of the more prominent ones are (i) the evolution of virulence, (ii) life history theory, and (iii) the generalism-specialism dichotomy. All involve purported tradeoffs between traits that define the evolvability and constraint of virus-associated phenotypes.

As the UK NHS faces growing challenges of the ageing population and escalating healthcare costs, we examine the role of the general internal medicine (GIM) consultant in inpatient medicine. We argue that the reintroduction of GIM could provide a sustainable and valuable contribution to high-quality, cost-effective inpatient care drawing on parallels from the GIM hospitalist model in the USA. Additionally, GIM expansion could contribute to improved outcomes for complex medical patients and surgical patients, as well as safety and quality initiatives.

Objective: To identify generalism-related competencies that medical students in Japan should acquire in order to provide comprehensive care for patients.

Methods: The team responsible for developing the new 'Generalism' section of the 2022 revision of the Model Core Curriculum for Medical Education in Japan (MCC) consisted of nine members from diverse medical backgrounds across Japan. We adopted pragmatism paradigm and analyzed to identify decision-making processes using a qualitative document analysis.
