Persistent Muellerian duct syndrome (PMDS) is a rare form of male pseudohermaphrodism without the feature of ambiguous genitalia. We present a case of PMDS with transverse testicular ectopia (TTE).

Background: Persistent mullerian duct syndrome (PMDS) is a rare, autosomal recessive disorder. It is a form of male disorder of sexual differentiation in which mullerian duct structures are present in male phenotypes and 46XY karyotypes. The treatment of persistent muellerian duct syndrome with undescended testis usually followed is orchiopexy without excising or partially excising the muellerian duct structures. Since the vas and vessels are intimately adhered to the fallopian tubes and body of the uterus, excision of muellerian duct structures is associated with devascularisation or injury to the vas. Following several reported incidences of malignancies in this residual muellerlan duct structures, we decided to excise them along with orchiopexy in this patient.

Absence of antimuellerian hormone or defective functioning of antimullerian hormone type II receptors give rise to a type of male pseudohermaphroditism known as 'persistent muellerian duct syndrome'. Such patients are phenotypically male with 46 XY karyotype and well developed muellerian duct derivatives which may be intra-abdominal or may herniate in inguinal region giving rise to a condition called as 'hernia uterine inguinale'. Cryptorchidism, transverse testicular ectopia and hypospadias are few anomalies associated with this syndrome.
