Background: Prior research suggests that patients may be entered into clinical trials with different electrocardiographic (ECG) findings than specified by study protocol criteria; the extent and impact of this variability in a large-scale trial have not been previously described.
Methods: We evaluated the relationship between case report form (CRF) categorization of the admission ECG and a Core Laboratory and subsequent outcome in a retrospective analysis of a trial of patients with acute ischemia and a broad spectrum of ECG changes (the GUSTO-IIb trial).
Results: In 11,037 patients with CRF information and an interpretable ECG, there was agreement in 89.1% of ST-elevation and 81.9% of non-ST-elevation cases. Among patients designated as having no ST elevation on the CRF, 1-year mortality rates were significantly higher in the subgroup of patients with Core Laboratory-determined ST elevation as compared with those where both the CRF and Core Laboratory classification were in agreement (8.8% vs 6.8%, P = .0093). Among patients designated as having ST elevation by the CRF, 1-year mortality rates were similar in both the subgroup of patients with and without Core Laboratory agreement (7.7% vs 8.2%, P = .72).
Conclusions: These findings have important implications for clinicians in routine practice because even a simple evaluation (presence or absence of ST elevation) on the admission ECG was often discordant and was associated with adverse clinical outcome.

Affected by weakening effect of water in the goaf, the bearing capacity of coal pillar reduced, and coal pillar rock burst is prone to occur, which is a serious threat to mine safety in production. In order to study the equivalent width and stability of coal pillar in water-rich coal seam, taking the section coal pillar of a working face as the research object, combined with laboratory test, theoretical analysis, simulation and engineering practice, the stress, elastic core area width, damage degree and energy accumulation of 36 m water-immersed coal pillar and 26 m, 28 m, 30 m, 32 m, 36 m unimmersed coal pillars are analyzed. Irreducible water saturation is an important factor affecting the development effect of low permeability reservoir. Using the self-developed ultrasonic generator, kerosene was used as simulated oil, the natural low-permeability siltstone cores with different physical properties in Zhongyuan Oilfield were selected for indoor oil displacement experiment, and the effect of ultrasonic action on the saturation of irreducible water in low-permeability reservoirs was evaluated. It was found that ultrasound can further reduce the saturation of irreducible water on the basis of oil flooding.

Generalized pustular psoriasis (GPP) is a rare human autoinflammatory disorder with life-threatening systemic effects. Keratinocyte-derived interleukin (IL)-36 signaling has been identified as a key mediator of immune response in the skin of affected individuals. Recognition of various mutations along the IL-36 axis and the downstream nuclear transcription factor κB (NF-κB) signaling have established GPP as genetically, immunologically, and histopathologically distinct and amenable to immunomodulation, which is epitomized by the recent success of IL-36 antagonism.
