In patients with pancreatogenic phlegmons lesions of the retroperitoneal fat were divided into four quadrants. The fat lesion was detailed with the help of ultrasonic examination, computed and magnet-resonance tomography. Operative interventions were fulfilled according to the lesion volume. From 314 patients with infected forms of necrotizing pancreatitis 209 patients with pancreatogenic phlegmons were chosen. They were operated on. Minimally invasive and endoscopic techniques were used in 116 patients, traditional methods were used in 93 patients. The factors increasing the effectiveness of treatment were the giving up of early operative interventions, adequate drainage of the purulent focus using minimal accesses. Lethality after minimally invasive methods was 11.2%, after traditional treatment - 29.0%.
