Unlabelled: Community-acquired methicillin resistant (CA-MRSA) strains are increasingly replacing hospital-acquired MRSA (HA-MRSA) strains in hospitalized patients leading to poor clinical outcomes. Hence, this study aimed to characterize clinical isolates of MRSA (HA-MRSA and CA-MRSA) and to understand their clonal origin. We report the first case of fatal community-acquired pneumonia with concomitant bloodstream infection caused by the ψUSA300 strain of methicillin-resistant Staphylococcus aureus (MRSA). The isolate was positive for Panton-Valentine leukocidin (PVL) gene, with a unique 12-base pair deletion in the ccrB2 gene, differentiating it from the canonical USA300 strain. Despite aggressive therapeutic interventions, the patient developed multiple complications, including septic shock, which ultimately proved fatal.

This study presents surveillance data from 1 July 2003 to 30 June 2023 for community-associated methicillin-resistant (CA-MRSA) notified in the Kimberley region of Western Australia (WA) and describes the region's changing CA-MRSA epidemiology over this period. A subset of CA-MRSA notifications from 1 July 2003 to 30 June 2015 were linked to inpatient and emergency department records. Episodes of care (EOC) during which a positive CA-MRSA specimen was collected within the first 48 hours of admission and emergency presentations (EP) during which a positive CA-MRSA specimen was collected on the same day as presentation were selected and analysed further.
