Background: Division of the rectum following total mesorectal excision (TME) using intracorporeal stapling devices is technically difficult due to their width and limited roticulation. More than one cartridge is often required and resultant wedging of the stump may be associated with an appreciable leak rate.
Methods: Three-dimensional reconstruction was performed of CT and MRI images from the lower abdomen of six patients undergoing laparoscopic TME using the Amira software environment. The stapling device was virtually reconstructed by in-house developed software, superimposed over the point of division of the rectum and the site of skin entry identified.
Results: The 45 degrees angulation of available roticulating stapling devices precludes perpendicular division of the rectum following laparoscopic TME. The optimal angulation for transverse rectal stapling varied between 62 degrees and 68 degrees .
Conclusion: A roticulating stapler with minimum angulation of 65 degrees would achieve transverse division of the rectum following laparoscopic TME.

Background: Lumen-apposing metal stents (LAMS) are utilized in a wide range of therapeutic gastrointestinal applications. We present our experience with LAMS-assisted drainage of complex pelvic abscesses at a large safety-net hospital.
Methods: EUS-guided LAMS placements for pelvic abscesses from July 2020 to June 2024 were analyzed.

Introduction: Cancer has become a public health problem worldwide, affecting individuals of different age groups, including children, young adults and older patients.
Objective: To determine the clinical profile, staging and standard of oncological treatment of the 10 most frequent primary sites of cancer in young patients (< 60 years) vs older patients (≥ 60 years old) diagnosed between 2000 and 2019 in Brazil.
Materials And Methods: This cross-sectional study used data from the secondary database of the Hospital Cancer Registry, available on the web www.

Anorectal neuropathy causes anorectal dysfunction, yet it is poorly recognized. This stems from both a lack of understanding of the extrinsic and intrinsic innervation of the anorectum and tools for evaluation of neuronal function. Our objective was to provide an improved understanding of the neuronal networks of the anorectum and discuss its functional significance.

Colorectal cancer (CRC) is a form of cancer that impacts both the rectum and colon. Typically, it begins with a small abnormal growth known as a polyp, which can either be non-cancerous or cancerous. Therefore, early detection of colorectal cancer as the second deadliest cancer after lung cancer, can be highly beneficial.
