The Medicare Prescription Drug, Improvement, and Modernization Act (MMA) provides economic incentives that favor health plans over traditional fee-for-service (FFS) Medicare. This reflects an ideological preference for private plans rather than government-administered pricing and recognition that private plans can use tools effectively to improve quality. However, enrollment projections indicate that FFS will continue to attract the majority of beneficiaries for years to come. We argue that MMA's contractor reform provisions create the opportunity to build critical FFS infrastructure, and contractors have the potential to encourage quality and manage utilization to compete with private plans in a modernized Medicare.

However, deep learning faces generalization challenges due to tumor variability even though measurements and segmentation of VS are essential for growth monitoring and treatment planning. Therefore, we introduce a novel model combining two Convolutional Neural Network (CNN) models for the detection of VS by deep learning aiming to improve performance of automatic segmentation.

Assertive Community Treatment (ACT) is a community-based, multidisciplinary mental health treatment model with improved housing stability as a treatment goal. We know little about factors contributing to housing stability among ACT participants with co-occurring serious mental illness and substance use disorders, who account for 30% of the ACT participant population. Informed by the behavioral model of health service use, the present study aimed to examine the relationship between housing stability and theoretically relevant factors.

Background: Hereditary Multiple Osteochondromas (HMO), previously known as Multiple Hereditary Exostoses (MHE), is a genetic disorder characterized by the formation of multiple, benign, exostoses (osteochondromas) growing from the metaphyseal region of long bones as well as from the axial skeleton. Lesions originating from the lumbar spine region are rare, and are most common growing from the posterior element of the vertebrae. HMO associated osteochondromas are difficult to treat due to continuous and incontrollable growth of these lesions and a lifetime risk for malignant transformation.
