Objective: To determine the safety, feasibility and effectiveness of stent expansion of hypoplastic aortic segments with pressure gradients in patients with arterial hypertension.

Design: Non-randomised prospective clinical trial.

Setting: Tertiary referral centre, congenital cardiac unit.

Patient Selection: 20 consecutive patients (median age 14.5 years, range 11.6-38.8 years) with arterial hypertension and a hypoplastic segment of the aorta. Seventeen patients had successful previous arch interventions in a coarctation site.

Interventions: Stent deployment in hypoplastic arch segments.

Main Outcome Measures: Gradient across the aortic arch; complications early and during follow up; residual hypertension.

Results: 23 stents were deployed: 13 in the cross and 10 in the isthmus. The mean gradient across the aortic arch decreased from 16 (SD 6) (median 17) to 3 (4) (median 1) mm Hg (p < 0.001). In a few patients a mild gradient persisted just distal to the left carotid artery due to residual orificial narrowing or acute angulation. No complications occurred during or after the procedure. During follow up of 2.2 years (range 0.2-4.8 years) arterial hypertension resolved in 10 patients and 10 required residual drug treatment with better control of blood pressures.

Conclusions: Pressure loss due to residual hypoplastic aortic segments can be treated effectively and safely with stent expansion. Some patients remain mildly hypertensive and require additional drug treatment.

Robust preclinical models of asymmetric ventricular loading in late gestation reflecting conditions such as hypoplastic left heart syndrome are lacking. We characterized the morphometry and microvascular function of the hypoplastic left ventricle (LV) and remaining right ventricle (RV) in a sham-controlled late gestation fetal lamb model of impaired left ventricular inflow (ILVI). Singleton fetuses were instrumented at ∼120 days gestational age (dGA; term is ∼147 days) with vascular catheters, an aortic flow probe and a deflated left atrial balloon.

Background: The precise etiology of hypoplasia of the posterior mitral valve leaflet (PMVL) remains incompletely elucidated; however, it has been hypothesized to stem from genetic mutations occurring during fetal development. Herein, we present the anatomical characteristics of the mitral valve and associated cardiac pathologies in patients with hypoplastic PMVL.

Methods: This single-center retrospective study involved patients who presented between 2015 and 2021 at a tertiary healthcare facility.

Congenital heart disease (CHD) remains a significant global health challenge, particularly contributing to newborn mortality, with the highest rates observed in middle- and low-income countries due to limited healthcare resources. Machine learning (ML) presents a promising solution by developing predictive models that more accurately assess the risk of mortality associated with CHD. These ML-based models can help healthcare professionals identify high-risk infants and ensure timely and appropriate care.

Background: The surgery on the craniocervical junction is associated with complex techniques that endanger the vertebral artery (VA), especially if there are some anatomical variations present, thereby increasing the risk of vascular injury, particularly during cervical decompression or instrumentation.

Case Description: A case of a 60-year-old female with progressive myelopathy and craniocervical junction malformation is presented. Key preoperative imaging findings included basilar invagination, C1 assimilation, and os odontoideum, along with VA anomalies such as a tortuous, hypoplastic left VA arising anomalously from the aortic arch and a right VA with a V2 segment forming a high-riding medial loop into the C2 vertebral body.

Semilunar valve regurgitation in single ventricular physiology is rare but portends a poor prognosis. Medical therapy is often ineffective particularly in the context of structural valve abnormality, and surgical valve repair or replacement can be associated with poor outcomes. We report an innovative use of a highly occlusive and conformable Gore Septal Occluder for transcatheter treatment of severe native aortic regurgitation in a symptomatic patient with hypoplastic left heart syndrome and severe systemic right ventricular dysfunction, resulting in safe and complete occlusion of the aortic regurgitant orifice and clinical benefit.
