Background: Rhabdomyolysis is frequently associated with acute kidney injury (AKI). Due to the nephrotoxic properties of myoglobin, its rapid removal is relevant. If kidney replacement therapy (KRT) is necessary for AKI, a procedure with effective myoglobin elimination should be preferred.

Idiopathic inflammatory myopathies represent a spectrum of autoimmune disorders primarily characterized by muscle inflammation. While renal involvement in idiopathic inflammatory myopathies has historically been considered rare, recent findings indicate a prevalence of approximately 21-23%. Renal manifestations in idiopathic inflammatory myopathies are generally secondary to acute renal injury from rhabdomyolysis or, more rarely, occur through autoimmune mechanisms leading to glomerulonephritis.

Purpose: The risk of exercise-induced rhabdomyolysis, followed by abrupt creatine kinase (CK) augmentation, associated with carnitine palmitoyl transferase II (CPTII) deficiency causes patients to abstain from physical training. However, the exercise adjustment to the disease-induced metabolic impairment, accompanied by a tailored nutritional and supplementation strategy, could make sporting activity feasible, even at a competitive level. Here, we report the case of an 18-year-old male basketball player at a competitive level diagnosed for CPTII deficiency after a rhabdomyolytic event.

Case Presentation: Here, we report a case of a 69-year-old female who was diagnosed with non-ST elevation myocardial infarction one month and was on regular medication before presentation with generalized weakness and decreased urine output.
