Background: Forearm stiffness can be caused by distal radioulnar joint (DRUJ) capsular contractures, which can occur after trauma such as a distal radius fracture. In this setting, a DRUJ capsular release may help improve forearm rotation, but the long-term functional outcomes remain unknown. The purpose of this case series is to investigate the short-term improvement in total pronosupination arc range of motion and long-term patient-reported outcomes (PROs) after DRUJ capsular release.

At the distal radioulnar joint (DRUJ) and the ulnocarpal joint several anatomical structures are in a limited space. Therefore, the operative approach has to consider the pathology to be treated. The DRUJ may be approached from the dorsal or the palmar side.

Locking of the distal radioulnar joint (DRUJ) induces restriction of the active and passive motion of pronation or supination of the forearm. Diagnosis of DRUJ locking due to a triangular fibrocartilage complex (TFCC) tear is sometimes difficult. A 15-year-old female suffered from restriction of forearm supination after cast immobilization for the treatment of an elbow injury.

We report a case of locking of the distal radioulnar joint (DRUJ) due to interposition of a flap of the palmar radioulnar ligament (RUL) torn from the triangular fibrocartilage complex (TFCC). We released the blockage by elevating the flap and partially resecting a portion of the triangular fibrocartilage (TFC) arthroscopically.
