As life expectancy continues to rise, the number of geriatric patients will increase and the percentages of geriatric patients seen in the emergency department will reflect those numbers. Emergency physicians are responsible for making immediate diagnoses and initiating expeditious treatment. Infectious diseases in the elderly are more prevalent, challenging to diagnose, and are associated with greater morbidity and mortality than with the younger patient population.

Aspiration pneumonia is a serious problem in the elderly due to weakened swallowing reflexes or underlying gastroesophageal reflux disease (GERD). This can lead to acute respiratory distress syndrome (ARDS), which can become life-threatening, sometimes requiring extra corporeal membrane oxygenation (ECMO) support. Lung transplantation is a possible therapeutic option for patients with no signs of lung recovery despite prolonged ECMO support.

Rationale & Objective: Engaging patients with advanced chronic kidney disease (CKD) in goals of care (GOC) conversations is essential to align life-sustaining treatments with patient preferences. This pilot study described the feasibility of engaging older Veterans with advanced CKD in GOC conversations via telehealth by (1) comparing patient characteristics, including life-sustaining treatment note completion rates and preferences by visit modality, and (2) exploring Veteran and clinician perspectives surrounding telehealth GOC conversations.
Study Design: Mixed-method convergent design including a prospective, quantitative observational cohort analysis (n = 40) and qualitative, semi-structured interviews with 4 clinicians and 11 Veterans.

Objective: To investigate the alterations in the Geriatric Nutritional Risk Index (GNRI) and bone morphogenetic protein 2 (BMP-2) levels and identify associated factors in older adults with delayed union of osteoporotic thoracolumbar spine fractures.
Methods: From June 2021 to June 2023, 139 elderly patients with osteoporotic thoracolumbar spine fractures were selected and divided into a delayed group and a normal group according to the fracture healing status at 6 months postoperatively. GNRI and BMP-2 levels were assessed in both cohorts.

The novel amyloid-beta, p-Tau, and neurofilament light chain (ATN) classification scheme has become a promising system for clinically detecting and diagnosing Alzheimer's disease (AD). In addition to its utility in Alzheimer's diagnosis and treatment, the ATN framework may also have clinical relevance in identifying non-Alzheimer's pathologies. In this study conducted at Broadlawns Geriatric and Memory Center, 92 amyloid-negative profiles out of 182 patients with an ATN framework were categorized into subjective cognitive impairment (SCI), non-amnestic mild cognitive impairment (non-amnestic MCI), amnestic MCI, Alzheimer's dementia, vascular dementia, mixed dementia, unspecified dementia, or other memory changes based on diagnoses written in the chart.
