AI Article Synopsis

  • Sarcoidosis is a chronic disorder that significantly affects the health-related quality of life (HRQL) of patients, particularly through respiratory issues like bronchial obstruction.
  • The study evaluated 129 patients with confirmed sarcoidosis using the St. George's respiratory questionnaire (SGRQ) and found that various clinical factors, such as gender and disease stage, influenced HRQL scores.
  • Results indicated that the SGRQ is a valid tool for assessing HRQL in sarcoidosis patients, but more research is needed to confirm its consistency and responsiveness in this group.

Article Abstract

Introduction: Sarcoidosis is a chronic multisystem disorder which has a great impact on health-related quality of life (HRQL). Patients with sarcoidosis very often present with bronchial obstruction. The purpose of this study was to assess the impact of respiratory disorder on general HRQL using St. George's respiratory questionnaire (SGRQ).

Material And Methods: We examined 129 patients with histologically proven sarcoidosis. All patients underwent clinical examination, chest X-ray and spirometry, and in all skin prick tests and sera ACE and IgE were determined. Afterwards, they filled out SGRQ, and scores were compared with clinical and radiological findings, and also with serum markers of the disease.

Results: Mean activity score values were significantly lower in men, in patients with newly diagnosed disease, and in patients with elevated ACE. Patients with extrathoracic disease had significantly higher scores. Clinical course showed significant correlation with impact and total score, and the number of relapses with the activity score. SGRQ scores significantly correlated with serum IgE and with most spirometric parameters.

Conclusion: Our data revealed a high validity of SGRQ in patients with sarcoidosis and a great influence of respiratory disorders on general health-related quality of life in these patients. However, reproducibility and responsiveness of SGRQ is yet to be established in patients with sarcoidosis.

Sarcoidosis is a systemic granulomatous disease of unknown etiology, primarily affecting the lungs and the lymphatic system. Its diagnosis is challenging, and in many cases, it requires histopathological confirmation through the identification of non-caseating granulomas. The presented case illustrates its diagnostic complexity and highlights a rare, delayed complication associated with endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA).

Gastrointestinal cancer incidence after lung transplantation in sarcoidosis patients.

Ann Gastroenterol

Center for Advanced Therapeutic Endoscopy, Porter Adventist Hospital, Centura Health, Denver, Colorado (Douglas G. Adler), USA.

Background: The risk of gastrointestinal (GI) cancer after lung transplantation (LTx) in sarcoidosis patients is not well defined. Given the cancer risks linked to sarcoidosis and organ transplantation, this study investigated the incidence of GI malignancies (DNM), comparing LTx recipients with sarcoidosis or idiopathic pulmonary fibrosis (IPF).

Methods: We analyzed data from the United Network for Organ Sharing registry, including adults with sarcoidosis or IPF who underwent LTx between May 2005 and December 2018.

Background: Preceding studies of environmental and occupational risk factors of sarcoidosis yielded inconsistent findings.

Aims: We aimed to ascertain the occupational risk factors for sarcoidosis in a case-control study.

Methods: A total of 237 sarcoidosis patients with a histologically confirmed diagnosis were matched with 474 controls for sex and age (median 49, interquartile range 37; 60 years) recruited from the university hospitals and outpatient centres in Belarus, Kazakhstan and the Russian Federation.

Background: We report a successful wound treatment of a chronic ulcer with bone exposure using a somehow forgotten technique of creating burr holes into the bone. Most clinics would promote flap surgery to cover wounds with bone exposure, however, in some cases invasive surgery is not mandatory. We bring up an alternative treatment for such cases.
