The wrap-around flap became the most popular technique for reconstruction of an amputated thumb, because it can create an aesthetic and functional replica of missing thumb without losing any toes. It provides length, stability, and adequate sensibility for a functional pinch and grasp. Nevertheless, long-term results revealed complications of donor site and reconstructed thumb. In this article, we describe our recent version of the wrap-around flap technique and discuss its complications and ways of preventing them.

For amputation of the thumb in any age group, microsurgical replantation is the gold standard over other osteoplastic thumb reconstruction methods as it restores the form, function, and cosmesis of the thumb better. In the osteoplastic reconstruction of the thumb, usually, a pedicled groin flap or a reverse radial artery forearm flap is used to provide the soft tissue cover, and each of these flaps has its own set of merits and demerits. The reverse radial artery forearm flap can be used as a fascial or fasciocutaneous flap in an islanded or peninsular form.

Background: Complex injuries of the hand frequently result in loss of essential functional features. Common reconstructive procedures for soft tissue defects of the thumb or phalanges are locoregional flaps like Moberg-, Foucher-, Cross-Finger- or Littler flaps. We report the results of using a fibula-sided digital artery pedicled flap from the great toe to cover the second toe free flap donor site, which avoids delayed wound healing, and prevents pain and skin ulceration. This study included 15 patients who had second toe wrap-around free flaps to reconstruct thumb and finger defects. All 15 pedicled flaps used to cover the defect healed uneventfully.
