Donepezil, a selective acetylcholinesterase (AChE) inhibitor, has been shown to reduce intraocular pressure (IOP) in ocular normotensive rabbit eyes. The aim of this investigation was to evaluate the effect of oral donepezil on IOP and pupil diameter after mid-term oral treatment in normotensive persons. Thirty-two newly diagnosed Alzheimer patients with normal IOP and no further antiglaucomatous treatment were included in the study. IOP and pupil diameter were evaluated before and 4 weeks after daily intake of 5 mg donepezil. IOP and pupil diameter were significantly lower/smaller after 4 weeks of treatment. The mean IOP of all 63 eyes was 14.1 mmHg before and 12.8 mmHg after treatment (8.8% reduction). Mean pupil diameter constricted from 3.9 to 3.6 mm (-7.4%). These findings show that donepezil, and, possibly, other selective AChE inhibitors, can potentially be used to treat glaucoma. They are also known to have neuroprotective effects in Alzheimer's, and, therefore, might have an additional therapeutic benefit.

Signal amplitudes obtained from retinal scanning depend on numerous factors. Working with polarized light to interrogate the retina, large parts of which are birefringent, is even more prone to artifacts. This article demonstrates the necessity of using normalization when working with retinal birefringence scanning signals in polarization-sensitive ophthalmic instruments.

Purpose: A relative afferent pupillary defect (RAPD) is a characteristic clinical sign of optic neuritis (ON). Here, we systematically evaluated ultrasound pupillometry (UP) for the detection of an RAPD in patients with ON, including a comparison with infrared video pupillometry (IVP), the gold standard for objective pupillometry.

Pupil responses are commonly used to provide insight into visual perception, autonomic control, cognition, and various brain disorders. However, making inferences from pupil data can be complicated by nonlinearities in pupil dynamics and variability within and across individuals, which challenge the assumptions of linearity or group-level homogeneity required for common analysis methods. In this study, we evaluated luminance evoked pupil dynamics in young healthy adults (n = 10, M:F = 5:5, ages 19-25 years) by identifying nonlinearities, variability, and conserved relationships across individuals to improve the ability to make inferences from pupil data.

Purpose: To evaluate clinical outcomes obtained after cataract surgery involving the implantation of a trifocal hydrophobic intraocular lens (IOL) and to determine if pupil size and the corneal aberrometric profile correlate to visual acuity at different distances.
Methods: 49 patients (98 eyes) underwent bilateral cataract surgery with the placement of FineVision HP IOLs for presbyopia and were assessed at 1- and 3- to 6-months post-surgery. Postoperatively, refraction, monocular and binocular uncorrected and corrected distance visual acuity (UDVA and CDVA), uncorrected intermediate visual acuity (UIVA), uncorrected near visual acuity (UNVA) and the binocular defocus curve were measured.
