A 10-year-old female presented with atraumatic bilateral mandibular swelling. Through imaging and exam she was found to have bilateral pneumoparotid. This is a rare cause of facial swelling that is primarily discussed in otolaryngology literature and relatively unknown in emergency medicine.

A 53-year-old man presented with the chief complaint of oral noises. No lesion could be observed in the oral cavity. Ultrasound detected several high-echogenicity areas inside the bilateral parotid ducts, and the initial stage of pneumoparotid was diagnosed.

Pneumoparotid is common in patients with lesions of the oral cavity who have diagnostic computed tomography (CT) with the "puffed cheek" technique. Although such observations are often noted, we could find few papers about the incidence in relation to oropharyngeal conditions. We present a retrospective series of 47/300 patients who developed pneumoparotid during multidetector CT examination of the oropharyngeal region to assess the incidence and any possible correlation with regional disease.

Problems/objectives: Pneumoparotid is a rare condition in which air is insufflated through Stensen's duct into the parotid gland. This causes mostly painless swelling in the parotid region.
Case Report: We report on a 7-year old girl presenting with painless unilateral swelling of the left parotid region, present for one month.
Bilateral pneumoparotid.
